How to Upload Your Vaccine Card to the Portal

Initial Vaccine

  1. Login to the portal at login.fwparker.org.
  2. Click on Resources.
  3. Click on Magnus.
  4. Once in Magnus, you will see your child(ren)'s names listed and just below in red, “You have requirements to complete on your To Do List.”
  5. Click on the orange “Complete Now” link, which opens the “To Do List.” 
  6. Locate the section that says "Has your child received the COVID-19 Immunization?" 
    Answer "Yes"
  7. You will then be asked, “Have you ALREADY obtained and completed the form "COVID-19 Immunization?”
    Answer “Yes, Submit.”

Boosters

  1. Login to the portal at login.fwparker.org.
  2. Click on Resources.
  3. Click on Magnus.
  4. Once in Magnus, go to their child’s list of requirements and update the “COVID Vax Card” requirement to “upload another”.
